  1. PSAL Medical Form completed and signed by student and parent/guardian. PSAL Medical Form
  2. PSAL Medical Form completed, signed, and stamped by a physician. 
  3. PSAL Parent Consent Form completed, initialed, signed, and dated by a parent/guardian.  PSAL Parent Consent Form.pdf 
  4. PSAL High-Risk Sports: Consent to Participate at Events and/or Venues  where COVID-19 Vaccine is not Required and Participate in Recreational Activities with Unvaccinated Athletes  PSAL Covid 19 consent

If you need support with completing medicals forms, you may visit the SUNY Downstate Medical Center on the dates and times that they are offer free medical and physical examinations.

A Message From SUNY Downstate Medical Center

Athletes, a free walk-in (no appointment needed) injury clinic will start on September 10th from 4-6 pm at 760 Parkside Avenue, Brooklyn. The Sports Medicine doctors from SUNY Downstate Health Sciences University and the Division of Sports Medicine put on the clinic. We offer this clinic to ALL athletes (yes, all, not just Football), and we follow the Boys Varsity Football schedule when we offer it. Here are the dates we will be offering it this Fall.

We have been providing this service for over 20+ years now  and look forward to continuing to do so.  We want the community to realize  we are here to help them and resource for all their needs.
